Henna While pregnant, lactating or anemic?

Henna is a natural product obtained from the crushed leaves of Lawsonia alba or inermis. Its application to skin and hair for cosmetics, healing and social occasions is a widely used practice. It usually carries no significant medical complications in a healthy individual.

However, henna in a patient with G6PD deficiency can cause serious medical complications, including severe hyperbilirubinemia and hemolytic anemia, due to its oxidative stress on the erythrocyte. The active integrant of henna, lawsone (2-hydroxy-1,4-naphthoquinone), has been linked to henna-induced hemolytic anemia (HIHA).

It may, however, be limited to people with weakened antioxidant defenses such as glucose-6-phosphate dehydrogenase (G6PD) deficiency.

Differential diagnosis

6-phosphogluconate dehydrogenase (6PGD) deficiency has similar symptoms and is often mistaken for G6PD deficiency, as the affected enzyme is within the same pathway, however these diseases are not linked and can be found within the same person.

TRIGGERS

Carriers of the underlying mutation do not show any symptoms unless their red blood cells are exposed to certain triggers, which can be of four main types: Foods (fava beans is the hallmark trigger for G6PD mutation carriers); certain medicines including aspirin, quinine and other antimalarials derived from quinine; moth balls (naphthalene).

Avoid henna if you have G6PD deficiency or have been advised to avoid Fava beans, are extremely anemic, or have citrus allergies. Consult a physician if you have any concerns or are pregnant and would like to use henna.

WARNINGS

Pregnant, lactating or anemic: Henna is not suitable for anyone who is G6PD enzyme deficient – if in doubt, check with your doctor. Please check with your doctor before you use henna.
